Terms and Conditions

I hereby acknowledge that I am familiar with all risks and hazards incidental to participation, and I further hereby assume all risks and hazards incidental to such participation including, but not limited, transportation to and from the activities, injuries, etc. In exchange for the valuable consideration participating in such activity, I hereby release, absolve, and agree to hold harmless 123totalfitness.com LLC.

I hereby authorize and consent to the use of the participant's visual image for appropriate purposes, including but not limited to: still photography, videotape, electronic and print publications and websites. I give this consent with no claim for payment. I understand that payment is due at time of registration unless other arrangements have been made and approved through management.

Cancellation Policy: We require a 24 hour notice if a scheduled session needs to be canceled. In the event that a client cancels without giving a 24 hour or greater notice; we will charge for that session.

If you have purchased the Sports Performance Classes, 123 Membership, or 123 & PB Membership; your card will be auto billed each month on the day you signed up. Example: (If you sign up on January 5th then your credit card will be auto billed on Feb 5th and so on). You can cancel at anytime however; we require a cancellation in the form of call, txt, or email 5 5 days prior to renewal.
